- Employment Identification Number (EIN) or Social Security Number (SSN)
- Supporting documentation for these business types:
- Sole Proprietor: Personal ID
- Limited Liability Corporation (LLC): Operating Agreement & Articles of Incorporation (active business registration with the Secretary of State required)
- Corporation: Articles of Incorporation & organization (paperwork you filed with the state when you first opened the business) + board minutes showing authorization
- Partnership: Partnership Agreement
- Government Entity or Unincorporated Association: Board minutes showing authorization
